Add a form to the activation process

From your 'My Hub' page (please see below)

On the left hand side of your account, you will see the side menu where you can select 'Forms'.

You will see a list of all the forms you have created 

For each form, you will be able to switch the toggle on to 'Show Form In Sign Up'
